Iowa Medical Marijuana Program

The first step when applying for a medical card in Iowa is scheduling an appointment with a certified marijuana healthcare practitioner (cannabis doctor). You can find out more about the Iowa Medical Cannabidiol Program here.

Before, patients had to download two documents and take them to their appointment; the Healthcare Practitioner Certification Form and the Patient Information Sheet. But, with QuickMedCards, patients benefit from telemedicine where they can have their marijuana evaluation online.

To avoid delays and improve the chances of approval, patients should upload their medical records. You can find out how to get your medical records and send them to your marijuana doctor online here.

Patients have the option of completing their registration online or via mail.

If patients decide to register through mail, they should download and complete the Adult Patient Registration Form. Then, include all the required documents and send them to the Iowa Department of Public Health:

Iowa Department of Public Health
ATTN: OMC
321 E. 12th Street
Des Moines, IA 50319-0075

Patients that provide their email address during their application process will get a temporary medical card to use before getting their permanent Iowa medical marijuana card in the mail.
