We recommend that you give the healthcare provider a copy of your medical history before your appointment, indicating your current medical condition. This is not required, but if you have it, you should upload any evidence of your qualifying condition or other medical records that will help the doctor determine if you qualify for medical marijuana. Examples include existing prescriptions to treat that condition, doctor notes, or other medical records. You can upload a document or a picture of a document.

If you arrive at your appointment without these documents, your marijuana doctor will most likely be able to request them on your behalf. However, this will prolong the process of obtaining the medical marijuana recommendation required to use cannabis in your state. Getting your medical records is simple and easy. Plus, it will help you get your medical marijuana card faster. 

Here are two easy steps to follow so you can get a hold of your medical records: 

1. Get In Touch with Your Doctor 

To get your medical records, most practices will need you to fill out a detailed form. Contact your primary care doctor’s office, or the office of the doctor who diagnosed you, and request a copy of the form. They should be able to send it to you through mail, fax, email, or have you pick it up in person. 

Should your doctor lack a standardized form, write a letter asking for these details:

  • Your full name plus your maiden name (if applicable)
  • Your social security number
  • State of birth
  • Residential address & telephone number
  • Email address
  • The requested record(s)
  • Specific dates you were under the doctor’s care
  • Option for delivery (pick up, fax, email, etc.)

What if I don’t have a Doctor? 

Your medical marijuana doctor might be able to help you locate an accessible clinic within your region that can perform an examination and diagnose you if:

  • You don’t have a primary care physician
  • You’re uninsured

2. Send Your Medical Records Before Your Meeting with the Doctor 

You can choose to upload your medical records when you first create your account at QuickMedCards before you book your appointment. If you’ve skipped this step and have already booked your appointment, you can easily upload your medical records prior to meeting with the doctor.

To do this, sign in to QuickMedCards and go to Medical Records. Next, you click the cloud icon to select the correct file from your device. You can select more than one file to upload, then click the Next button to save your files. If you need to add additional files go to Edit Profile and scroll down, here you will see the option to Add New Files.
