What’s Next After Getting A Recommendation?

    The Application Process Explained

Step 1: You can book an appointment with a licensed physician before you register on the state website.

After you receive your mmj recommendation, you must register as a patient in the state portal and pay the non-refundable state fee. Once you register and add your provider’s information, the state will automatically contact the provider to complete your online application.

Please note that the department requires your medical records to support the application.

Step 3: You must register on the state website in order to begin your application.
    Go to: Delaware.gov/mmp/patients

Click the registration link at the top right corner of the page, or scroll down and click the ‘New Program Participants’ button. Either way, it will lead you to the next page where you’ll need to type in your information, such as your first and last name, and your valid email address. You also need to assign a password for your account.

Step 4: Once you click on ‘Submit’, you will receive a confirmation link in your inbox, or you can check on your spam folder if it isn’t there. You will need to click on that link and it will redirect you to a login page. Once you’ve logged in, hover your pointer to ‘Applications’ and click on ‘Patient Application’. You will then need to fill out the whole application form and upload your photo.

Step 5: Under the ‘Physician’s Information’, type in the first three letters of your physician’s first and last name. (Ex. Physician’s Name: John Smith, for first name, type: joh and for last name, type: smi) Once typed in, click on ‘Match’ and the form will be pre-populated with your physician’s information.

Step 6: Complete the rest of the form. A certain percentage will reflect on your state portal so you have an idea of how much you’ve done in completing the form.

That’s it! You completed the entire process of applying for your medical mj card online.
